A certificate of criminal record is issued only upon the person’s direct application and on the basis of an identity document.
To obtain the relevant certificate, the following documents must be submitted to the Consular Section of the Embassy:

The citizen’s application (the application must include: the applicant’s surname, name, patronymic, place and date of birth, citizenship, residential address, and the series, number, date of issue, and issuing authority of the identity document). The application form is completed at the Consular Section;

A copy of the applicant’s identity documents (passport, ID card);

The application form for obtaining the document upon request, or a photocopy of the certificate with a QR code obtained via the “E-polis” mobile application;

A receipt confirming payment of the state fee.
Note: Each person may obtain a criminal record certificate only about themselves.
